If you need proof of immunization in the meantime, here’s what you do.
Visit Ontario’s On-line COVID-19 vaccination site (covid19.ontariohealth.ca) and verify your identity so that you can access your information.
You will need to supply details listed on your Ontario Health Card (photo version)- including your health card number, the two-letter version code, and the nine-character code on the back of your Health Card.
Provide your birthdate: the four-digit year, followed by two-digit month, and two-digit day.
Provide your postal code.
Press enter, and print-off or take a snapshot of your proof.
