Proof of eligibility is required to get a COVID-19 vaccine in Renfrew County and District.
The local health unit says everyone getting vaccinated needs to bring a printed and completed consent form.
Those over 80-years of age are asked to bring their health card, list of current medications and reading glasses or hearing aids if required.
Indigenous residents over the age of 55 are asked to bring their health card and proof of indigenous identity.
Priority health care workers who are eligible to get vaccinated are asked to bring along proof of employment.
The Ontario government has announced Phase Two of the vaccination plan in April will include adults over 60, individuals with specific health conditions and some primary caregivers, people who live and work in congregate settings, people who live in hot spots and certain workers who cannot work from home.
Ontario is expecting to vaccinate these groups from April to July 2021, depending on availability of
vaccines from the federal government.
