A collaboration of over 60 health system partners from the Arnprior, North Grenville, Carleton Place, Almonte and West Ottawa regions reached an important milestone today in their application to become an Ontario Health Team (OHT), with the Ministry of Health officially approving their application.
The move comes the same day as the Province gave its nod to the Network 24 Health Team, representing central and western areas of Renfrew County.
Although the Four Rivers Team got an earlier start, it went through three iterations before Queen’s Park was satisfied with the urban-rural balance and requested the Kemptville area be added.
OHTs, being formed across the province, are teams of health and community support services providers that are accountable for delivering coordinated care to a defined geographic population.
They are being developed as part of the government’s health system plan and People’s Healthcare Act, 2019.
They bring together health care services such as primary care, hospitals, public health, long term care, home and community care, and community support services under one umbrella to help people more easily get the right care, at the right time, in the right place.
In its first year, the new OHT will focus on improving mental health and addictions services.
The Four Rivers name may be transitional, as the OHT steering committee is currently seeking input from lived experience partners and providers on a new name that will represent the population.
A rule of thumb for Renfrew County residents is that communities from Braeside east fall within Four Rivers jurisdiction, whereas the Network 24 Health Team encompasses central and western Renfrew County.
